I don’t see any earnings from icon ads, push ads, smart wall ads and banner ads, but all these ads are appears in my app, even I clicked some ads in my app, because I wanted to test ads. I see only some earnings from AO2.

The Bundle 2 SDK only pays for the install and EULA opt in for new / unique users to the Airpush network (shown as the A02 ad type), it does not pay impression revenue for the ads. The benefit to this SDK is the payment up from the install and opt in giving you a $50 eCPM, and not having to be concerned with lifetime value of the user.

is required to implement icon ads, banner ads, push ads and smartwall? if yes, can I set minimal amount of these ads, I mean:
-1 icon ad per user per month,
-1 push ad in 7 day
-banner in only one place
-smartwall in only one place ?
It is required to implement all of the ad types within the app, but you can adjust the frequency of the icon and push ads if you wish. You can only have the banners and smart wall located in one place of the app.
